Chief Care Coordinator

Key Responsibilities:

  • Oversees the provision and development of clinical services under the Kwai Tsing District Health Centre
  • Prepare annual business plan and budget plan
  • Monitor the service schedule and publicity strategies for launching centre services
  • Set protocols for professional services and treatments
  • Provide supervision and coaching for care & service team

Requirements:

  • Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong
  • Degree in Nursing or equivalent
  • Minimum 15 years of working experience in patient care related nursing practice as Registered Nurse
  • Good command of both written and spoken English and Chinese
  • Strong interpersonal skill, good co-ordination and management experience
  • Organized, detailed, able to handle multi-tasks effectively and prioritize works to meet tight schedules

Officer

Duties:

  • Assist the Senior Manager as directed.
  • Responsible for administrative support of the association, including board meetings’ documents, funding proposals.
  • Responsible for Administrative and Accounting support of the association.
  • Perform full spectrum of general office administration and day-to-day operations, such as procurement, office supplies, repair and maintenance, and supervise supporting staff.
  • Coordinate operation and set up schedules.
  • Coordinate with internal staff and external parties, including vendors and local government authorities, etc.

 Requirements:

  • Diploma or degree holder, passed in HKCEE with Grade D or above or HKDSE with Level 2 or above in English and Chinese.
  • A minimum of 4-years relevant working experience, preferably in the social services sector.
  • Strong analytical mind, well-organized, strong interpersonal skill and self-motivated and be able to supervise a small team.
  • Excellent command of both written and spoken English and Chinese.
  • Proficient in PC knowledge: MS Office (including Word, Excel and PowerPoint).

Kwai Tsing District Health Centre(K&TDHC)

The Kwai Tsing Community & Health City Association (KTSCHCA) is currently recruiting high caliber candidate to join our team to work for the Kwai Tsing District Health Centre (K&TDHC) which is a new 3-years pilot programme commissioned by the Food and Health Bureau of HKSAR that aims to help everyone to learn to manage their health, wellbeing and care for each other through a warm, welcoming and smart DHC Health Hub and Service Network.

 

Care Coordinator (Nursing)

Key Responsibilities:

  • Provide health assessment, screening, counselling, health education, and other nursing services in DHC Core Centre and Satellite Centres;
  • Provide health promotion and primary prevention programs;
  • Act as case manager of community rehabilitation programs;
  • Coordination of care among multi-disciplines in DHC and the DHC Network Service Providers;
  • Operate the clinical management system for clinical documentation and communication.

Requirements:

  • Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong;
  • Degree in Nursing or equivalent;
  • Minimum 5 years of working experience in related services as registered nurse;

Occupational Therapist (Part-time / Full-time)

Key Responsibilities:

  • To provide occupational therapy services to DHC clients, including assessment, treatment programme planning and implementation, related documentation, and communication;
  • Develop and provide group work, training, health promotion and patient education programmes that aim to enhance self-care and self-management skills;
  • As a team member of multi-disciplinary healthcare team;
  • To supervise a team including assistants, support staff, students and volunteers to ensure quality of service;
  • Communicate and liaise with Hospital Authority, network medical practitioners, professional groups and related parties.

Requirements:

  • Be registered as an Occupational Therapist in Part I or Part II of the Register of Occupational Therapists under the Supplementary Medical Professions Ordinance;
  • Have a valid practicing certificate issued by the Occupational Therapist Board of Hong Kong;
  • Practice as an Occupational Therapists for at least 5 years. Experience should cover at least 2 clinical areas;
  • 44 working hours per week, required to work irregular hours;
  • Required to work in the Kwai Tsing District Health Centre and the satellites centres;
  • May be required to work in another work location to meet service needs of Kwai Tsing District Health Centr

Administrative Assistant

Job Duties:

  • Responsible for the full spectrum of office administration and clerical support;
  • Maintain filing and document control system;
  • Sourcing and negotiating with suppliers or contractors on office supplies and maintenance;
  • Liaise with internal and external parties to ensure smooth daily operations;
  • Assist in ah-hoc tasks as required.

Requirements:

  • Diploma or above;
  • Minimum 2 years relevant working experience in office work;
  • Good command of written and spoken English and Chinese;
  • Proficiency in PC applications (e.g. MS Word, MS Excel and MS Powerpoint).
  • Strong interpersonal skill, good co-ordination and organization power.

Social Worker

Key Responsibilities:

  • Coordinate training, health promotion activities and assessment with the community stakeholders;
  • Coordinate and arrange patient empowerment programs and other community support programs;
  • Act as case managers of community rehabilitation and provision of counselling services;
  • Volunteers planning, training and development;
  • Develop social support network.

Requirements:

  • Registered Social Worker with degree in Social Work or above;
  • Minimum 3 years of relevant working experience in medical, community health / rehabilitation projects, elderly services and volunteers’ development;
  • Self-initiative, independent, mature, flexible and good interpersonal skills.

社區健康策劃員(臨時):

主要職責

  • 負責接待,會員登記和活動查詢
  • 協助專職醫護人員進行評估,諮詢和治療
  • 協助舉辦各種健康推廣活動及自我健康管理小組
  • 負責主中心及附屬中心的運作、服務及行政工作

入職要求
大專程度或以上,具備健康相關的專上教育,例如公共衛生、社區健康、營養學、老年學、體育運動科學等可獲優先考慮

中學程度並有兩年工作經驗,具備醫療或社福機構工作經驗,例如健康服務助理、物理/職業治療助理可獲優先考慮
星期六、公眾假期或下午需輪班工作
